Medicaid A&D Waiver services for clients aged 60 years and older have transitioned to the new Pathways for Aging program.

CARE Team has contracted with Humana to serve clients 60 and over.

Medicaid A&D Waiver services for clients under 60 years of age have transitioned to the Health and Wellness Waiver.
​
Under the Health and Wellness Waiver, Structured Family Caregiving providers are now able to provide services to minor clients whose caregiver is their parent or guardian. If you are in need of SFC services, please call or message us so that we may assist you through the transition.
